Geriatric Mental Health Teams (GMHT) 

GMHT Referral Form W-00733

  • GMHT are a team of health professionals (Geriatric Psychiatrist, Clinical Nurse Specialist, Psychiatric Nurses, Occupational Therapists and Social Workers) and one member of the team visits clients over age 65 in their home Monday through Friday
  • Provide in home assessment in the community and in personal care homes across the WRHA
  • GMHT assess memory, mood, mental health issues and emotional and behavioral complications of brain disease on a non-emergent basis
  • In addition to assessment, our service provides recommendations, short term intervention, consultation to care providers, education and support, and connection with resources.
  • Referrals can be made for people age 65 and older with first onset of mental illness or lifelong mental illness requiring specialized geriatric services
  • In order to complete a home visit, the Team requires client's name, address, telephone number, date of birth, and Manitoba Health Registration Number

Geriatric Program Assessment Teams (GPAT)

GPAT Referral Form W-00733

  • GPAT are a team of health professionals (Geriatrician, Nurses, Occupational Therapists, Physical Therapists, and Social Workers) and one member of the team visits the frail, elderly client age 65 or older in their home
  • GPAT also provide geriatric consultation for individuals age 65 or older in all six Emergency Departments across the WRHA
  • GPAT assesses many areas of your health and day to day functioning including mobility/falls, activities of daily living (dressing, grooming, etc), toileting, memory, mood, medication management, and social supports
  • In addition to assessment, our service provides recommendations, short term intervention, consultation to care providers, education and support, and connection with resources
  • In order to complete a home visit, the Team requires client's name, address, telephone number, date of birth, and Manitoba Health Registration Number
