collaborate: education for health professionals


Participants will:

  • Develop an understanding of concepts of Collaborative Care across the care continuum
  • Describe how Collaborative Care concepts relate to the various components of the care continuum
  • Acquire a working knowledge of tools and strategies to advance the Collaborative Care initiative
  • Develop a plan for advancing their interprofessional Collaborative Care initiatives.

WHEN:  April 17 - 19, 2018

COST:  $250.00 per participant

LOCATION:  Radisson Hotel, Downtown Winnipeg                          

COLLABORATE CARE APPLICATION:        

WHO SHOULD BE NOMINATED:

  • interprofessional groups of two to four
  • people in positions or roles which would allow them to facilitate team change on an ongoing basis (formal/informal leaders, informal leaders, educators, or frontline staff)

EXPECTATIONS OF PARTICIPANTS:

  • Participants must be given time to attend not only the three day workshop, but also time within their daily work hours to advance their initiatives.  Leadership must be prepared to allow for this time.
  • Participants must meet with their leadership within 2 weeks of completion of the workshop to review progress on the initiative.
  • Participants are required to attend a 1/2 day workshop on Sept. 19th and the 1/2 day follow-up session on Nov. 27th to present work to date.
  • Participants will be invited to join the Interprofessional Collaborative Care Community of Practice.

EXPECTATIONS FOR LEADERSHIP AND EXECUTIVE: 

  • Approve and support the Interprofessional collaborative Care initiative
  • Identify and ensure alignment of the Collaborative Care initiative with strategic priorities
  • Support sustainability of the Collaborative Care Initiative by removing barriers, assisting participants with conflict management and promoting advancement of Interprofessional opportunities

SUCCESS FACTORS:

  • Do/will participants have the ability to facilitate change in the team/program/site they represent?
  • Is there willingness/interest within the team to move to more collaborative practice or evaluate their practice through a Collaborative Care lens?
  • Is there time/support for development and implementation of collaborative care initiatives by the participants following participation in the collaborate 2017 education session?
