Chronic Care
Chronic care services are for individuals who need specialized care due to a chronic health condition or who need more care than a PCH can accommodate.
There are two chronic care facilities in the city - Deer Lodge Centre and Riverview Health Centre. The fee for residential services in Chronic Care is called a per diem, or daily rate. It is subsidized and the cost is based on each individual's income.
What is the Long Term Care Access Centre?
The Long Term Care Access Centre is responsible for scheduling panel dates, ensuring clients have access to long term care services that are most appropriate to their needs and maintaining waiting lists for personal care homes/long term care programs.
