How can I report a critical incident?

Anyone can report a critical incident. If you believe such an event has occurred you should call the Critical Incident Reporting and Support Line (CIRSL).  A specially -trained counselor will gather information about the incident, guiding you though a series of questions. We know that this may be a stressful time and a difficult decision for you. Making the call is important and can lead to improvements in the health-care system.

Call anytime 24 hours a day at 788-8222 to report a critical incident.

You may remain anonymous. The counselor who takes your call can also provide confidential support if you’re feeling affected by the event.
