The Conceptual Framework provides important information about Winnipeg Integrated Services, including:

  • An overview of the vision, goals and design principles of integrated, community-based health and social service delivery

  • A definition of the Community Access Model and what constitutes an ACCESS Centre

  • Information on core health and social services that will be integrated through the integrated services
