
Healthy Aging Resource Teams
Healthy Aging Resource Teams work in the community to promote health,
increase awareness about injury and illness prevention, provide primary care and management of chronic diseases for adults age 55+.
Healthy Aging Resource Teams consist of two health care professionals such as a nurse, occupational therapist or dietitian. Together, they provide health services and community support for older adults living in the River East, St. James/Assiniboia and Downtown community areas. Health clinics are held at a variety of community sites including apartment blocks, malls, seniors centres and churches.

- health promotion;
- chronic disease management;
- injury and illness prevention;
- providing programs to enhance the quality of life;
- encouraging self care, independent living and choices for one’s own best health;
- building resources for a healthy, welcoming community for older adults.
